Medical Release/Emergency Cards
These cards must be returned to the center as soon as possible after enrollment. This allows the staff to seek medical attention for your child in case of an emergency. If the parent has a specific medical center at which they prefer the child be treated in case of an emergency, a letter specifying the facility needs to be written to the center.
Please remember to update Northfield Montessori regarding any changes in home, work or medical phone numbers.
